Vendor portal

The Vendor Portal is currently available for Surgical Services requests only.

Vendors requesting the use of equipment, instrumentation, supplies, and/or medications at Columbus Community Hospital Surgical Services are required to complete the vendor request form below. 

Completed submissions would then be emailed to vendorrequest@columbushosp.org.

Incomplete requests will not be considered.
