When you receive a diabetes diagnosis, your life changes forever. You may be feeling overwhelmed by all the ways in which you must alter your diet, your exercise habits and your lifestyle.
CCH’s Diabetes Self-Management Program can help you become more prepared for how different your life is going to be. It provides comprehensive diabetes education for people who are newly diagnosed with diabetes, as well as those with a long-standing history of diabetes.
The class covers blood glucose monitoring, nutrition, exercise, lifestyle changes, medications, risk reduction and diabetes self-management. When we assess you at the beginning of the program, we will teach you how to monitor your blood glucose and examine your feet on a regular basis.
The six-hour course includes a one-hour individual appointment and two separate two-and-a-half-hour group sessions.
