Columbus Community Hospital's Patient Navigation Program can help.
In this free program, oncology nurse navigators and oncology patient family resource specialists guide and support you through your entire cancer treatment and recovery process.
Nurse navigators can:
- Help provide diagnosis information.
- Help you understand your treatment options.
- Discuss warning signs or symptoms during treatment.
- Collaborate with members of your medical team.
- Help you and your family connect with community resources such as Thrive Cancer Rehabilitation Care, support groups, survivorship classes and legal aid.
A patient family resource specialist helps with other parts of your life, such as your self-image, work, family and friendships. Patient family resource specialists can provide:
- Complementary therapy and referrals.
- Community resource referrals.
- Financial/insurance referrals.
- Patient and family counseling referrals.
- Advocacy.
- Social Security/disability referrals.
- Legal service referrals (advance directive, living will and more).
- Transportation referrals.
- Home care, assisted living and nursing home referrals.
