Do you know a nurse who deserves to be recognized for providing outstanding clinical care with compassion? The DAISY Award is an international program that rewards and celebrates nurses’ extraordinary clinical skill and compassionate care. Columbus Community Hospital is proud to be a DAISY Award Partner, and we recognize one of our nurses with this special honor each quarter.
How to nominate a nurse
Simply fill out a nomination form by clicking on the “Nurse Nomination Form” link below and telling us how this nurse made an extraordinary impact on you or your family.
Patients, visitors, nurses, physicians and staff are welcome to nominate a deserving nurse.
CCH presents a DAISY Award each quarter in February, May, August and November. Nominations that are received after the winner is chosen for that quarter will go toward the next DAISY Award.
