Under the direction of the East Central District Health Department (ECDHD), a Comprehensive Community Health Needs Assessment has been devised for the four counties in the Health Department’s district, which includes Boone, Colfax, Nance and Platte Counties. The CHNA serves as a reference for selection of strategic issues for the four non-profit hospitals within the district.
This assessment was conducted in partnership with multiple agencies within the district and is the basis for the Community Health Improvement Plan (CHIP).
It is the goal of the CHNA to describe the health status of the population, identify areas for health improvement and, determine factors that contribute to health issues, and identify assets and resources that can be mobilized to address public health improvement.
This assessment will be updated and revised every three years, thus providing communities with up-to-date data to evaluate progress made towards identified health priorities, and for the selection of new ones.
