Community Health Needs Assessment

Under the direction of the East Central District Health Department (ECDHD), a comprehensive Community Health Needs Assessment (CHNA) has been created for the four counties in the health department’s district, which include Boone, Colfax, Nance and Platte counties.

The goal of the CHNA is to describe the health status of the population, identify areas for health improvement, determine factors that contribute to health issues, and identify assets and resources that can be mobilized to address public health improvement.

The CHNA was conducted in partnership with multiple agencies within the district, and it serves as a reference for selection of strategic issues for the four nonprofit hospitals within the district. It is also the basis for the Community Health Improvement Plan (CHIP).

The CHNA will be updated and revised every three years, thus providing communities with up-to-date data to evaluate progress made toward identified health priorities, and assist with the selection of new ones. This, in turn, leads to updated Community Health Improvement Plans, which are also released every three years in conjunction with the updated CHNAs.
