Every three years, a comprehensive Community Health Needs Assessment (CHNA) is created for the four counties in the East Central District Health Department (ECDHD), which includes Boone, Colfax, Nance and Platte counties. It is created by the ECDHD, CCH and other agency partners.
The assessment describes the health status of the population, identifies areas for health improvement, determines factors that contribute to health issues, and identifies assets and resources that can be mobilized to address public health improvement.
The CHNA also serves as a reference for selection of strategic issues for the four nonprofit hospitals within the district, and is the basis for the Community Health Improvement Plan (CHIP), which guides health improvements in Platte County.
The Platte County CHIP is updated and reviewed on a quarterly basis during the three-year duration of the plan.
