Step One (Personal Information)

Let your friend or family member know you are thinking about them while they are visiting the hospital by sending an eCard greeting. Please fill out the form below and your friend or family member will receive your special greeting delivered directly by our Volunteers. This FREE service is available for any inpatient currently staying at Columbus Community Hospital.

ECards are delivered by volunteers within 24 hours of submission, Monday through Friday, (except national holidays). ECards received after noon on Friday will not be delivered until the following Monday

IMPORTANT
When you are filling out the form above, please make sure you include the patient's first and last name so our volunteer staff can make sure your eCard is delivered to the correct individual. We cannot deliver eCards to patients in our Emergency Room or patients who are receiving outpatient surgery. Please do not include confidential information in your message.
