Group Basic Life Insurance

Base life insurance amount equal one time the annual salary. Term type insurance.

Voluntary Life Insurance

First of the month, following date of hire (must be scheduled to work 20+ hrs per week). Term type insurance. Guaranteed amount if enrolled during first 30 days of eligibility.

Group Long-Term Disability Insurance

First of the month, following date of hire (must be scheduled to work 36+ hrs per week).

After 30 day wait, plan pays 66 2/3% of base salary.

Dental/Vision Insurance

Flexible Spending Accounts
Staff
Within the first 30 days of employment (must be scheduled to work 20+ hrs per week). Staff elects monies to be deducted, tax free, for payment of dependent care or unreimbursed medical expenses.
Paid Time Off (PTO)

Accrual starts first day of employment. Available immediately as accrued.  Elevated accrual rate for employees after completion of 7 or more years of service. 

Funeral Time

Upon employment.

Time off with pay may be granted for bereavement. Temporary, PRN and PT employees scheduled to work less than 20 hours per week are not eligible for paid Bereavement Leave.

Retirement Fund

Within the first 30 days of employment or quarterly thereafter for employee contribution.

Staff contributes between 1% and 100% of income on a tax sheltered basis. CCH match is 200% up to 4% max, after 12 months, 1,000 worked hours, 21 years old.  Employees with 15 or more years of service are eligible for 5% Employer Match.
